The Supreme Court of Ohio adopted Rules of Superintendence for the Courts of Ohio 44-47 effective July 1, 2009. |
| Pursuant to Rule 45(D) of the Rules of Superintendence for the Courts of Ohio, parties and their attorneys shall omit personal identifiers, as defined in Rule 44(H) of the Rules of Superintendence for the Courts of Ohio, from all case documents filed with the Clerk of Courts whether filed in paper or electronic form.

| Rule 45(D) also authorizes the Clerk of Courts to provide a standard form for parties to use to reference the omitted personal identifiers. The forms for each particular Court can be found below and also under each division's forms section.
